This form is for MLHPP providers to submit external educational credits to qualify for their value-based educational requirement. In order for external education to meet this requirement a CME credit certificate must be submitted to MLHPP@mlhs.org (please see below for submission instructions). Any educational program should be value-based in nature and should contain components of the following definition of value-based:

Value-based healthcare is a healthcare delivery model that prioritizes value over volume in which providers focus on helping patients improve their health, reduce the effects and incidence of chronic disease, and live healthier lives in an evidence-based way. These models require accurate capture of a patient’s risk to determine expected total cost of care. Value-based care delivery structures strive towards the triple aim of reducing the total cost of care while improving patient experience and the overall quality of a patient’s health. Utilizing interdisciplinary approaches to communication, collaboration, and care coordination, value-based care emphasizes quality and health outcomes. It aligns care delivery with how patients experience their health and reconnects clinicians to their purpose as healers.

Some examples of value-based care topics include, but are not limited to:

  • Accuracy of risk coding and documentation (i.e. HCC coding)
  • Advanced care planning
  • Decreasing avoidable admissions and readmissions
  • Improving discharge to home
  • Reducing unnecessary variation in care
  • Chronic disease ambulatory care guidelines
  • Appropriate utilization of care
  • Patient-centered care
  • Optimizing Rx drug spending
  • Closing care gaps (e.g. cancer screening, diabetes care, immunizations)
  • Consideration of relevant patients for Palliative care consultation
  • Appropriate use of hospice at end of life
  • Referrals to PCPs for patients without them
  • Order set use and compliance
  • Appropriate utilization of home care
  • Combating provider burnout
  • Care management for ED high utilizers
  • Safe disposition alternative to admission
  • Inclusion of care coordination and social work

Your CME certificate must be submitted to MLHPP@mlhs.org for this program to be considered for meeting the MLHPP educational requirement.
